I am writing this unsolicited letter to you in recognition of the exemplary work done by Mr. Ross Maultasch and his staff at the Windsor Garden Care Center of Fullerton.

My mother, Virginia Bonsangue, 85, has been a patient there since early December following abdominal surgery and other related issues. I met Ross when she had a concern regarding one of the night staff members during her first week. Ross took the concern seriously addressed it in a professional and timely manner.

Our experience at each level, from admissions to RN/CFN to PT/OT, has proven to be positive and effective. I am especially appreciative of the patience the medical staff have shown towards Virginia, who is a large person and not easy to handle.

As one who manages more than 80 part-time faculty members and teaching associates, I understand the commitment and time it must take to run a good Care Center. My family and I are deeply appreciative of Ross Maultasch’s effective leadership and genuine commitment, as well as that of the staff at WGCCF, to providing quality patient care.

Windsor Gardens Convalescent Center of San Diego

From the personal experience from a daughter of a resident who lives at Windsor Gardens Convalescent Center of San Diego, it is a homey place, just like a family. It is very bright, tastefully decorated and cheerful.

Windsor Gardens staff to resident ratio is excellent. That’s one ofthe first things my sister and I noticed when we toured Windsor Gardens for the first time, before placing our mother. There are nurses everywhere tending to all the residents with thoughtful caring and respect. There are times when a resident isn’t cooperative. That poses no problem for the nurses as they are always kind and patient.

From the moment you enter the front door you feel the warmth. It has two patios with lots of beautiful plants & trees. They have just recently added tables & chairs on the patios so visitors and residents can sit outside and enjoy the wonders of nature. The nursing staff and other staff members go the extra mile to make the resident’s ‘home’ a pleasant place to live. Each staff member knows all the residents by name and cares for them like they were their own family member.

The resident’s rooms are cleaned every day and also the bed linens are changed daily. The entire facility is ‘white glove’ clean, is well strives to maintain and has no disagreeable odor. The residents are served three tasty nutritious meals a day with snacks in between on request. Substitutions are available if the resident doesn’t care for their meal. No one needs to go hungry with these options.

There is always a fun activity planned each day. Sometimes even a field trip away from the facility. Saturday morning donuts and coffee are a favorite of many. They have birthday celebrations for their residents with special treats and balloons for the birthday person. Lots of events to keep the residents busy. Another favorite is music, whether it is with live musicians or recorded. They enjoy it all. Their barbecues on the patio are great with hamburgers & hotdogs and all the extras. We especially love the soft homemade oatmeal raisin cookies. Their Christmas dinner for residents and guests is outstanding.

The staff is great at keeping me informed, which is truly appreciated. Prior to being a resident of Windsor Gardens, my mother spent some time rehabilitating in two other South Bay facilities. Neither of these two facilities could hold a candle to the care she is receiving at Windsor Gardens. We consider it to be the best facility of its kind in the South Bay.

We are extremely pleased we found just the perfect home for our mother. Her daily nurturing and care has exceeded our expectations. We feel she is safe and emotionally and physically well cared for. In October of 2005, I came home one evening to find that my Mother had suffered a massive stroke. After five days in the hospital, we were told that she had to be placed in an acute care facility, most likely for the rest of her life. This is an extremely difficult decision to make as your concern for the care of your loved one outweighs all else. After visiting several facilities, we went to Windsor Gardens and were struck by the warmth and receptiveness of the staff. My brother and I decided immediately that this was the only choice we could make. After a short period of time, my Mother was placed on hospice care. The Windsor staff worked closely with the hospice workers to insure that my Mother was comfortable and peaceful during those last weeks. With 35 years of clinical experience as a Registered Nurse in multinational settings, Ms. Lillis joined the Windsor team in 2005. Designated California’s Nurse of the Year in 2000, she provides clinical oversight to the 34 facilities operating under the Windsor nameplate. A federal and state clinical regulatory expert, Ms. Lillis ensures that Windsor’s sterling reputation for quality patient care flourishes. Kevin joined Windsor as the Southern California Regional Director of Operations in 2010. Mr. Bowman became a licensed Administrator in 1992 and has worked in multi-state facility operations since 1998. His work experiences have included skilled nursing and sub acute facilities, in adition to assisted living and memory care programs. Mr. Bowman credits his grandmother for teaching him to recognize his purpose to serve others in his life’s work, and her memory keeps him passionate about positively enhancing the lives of residents, families and employees, every day.

Regional Director of Operations
As an Operations manager with 30 years experience with public and privately owned health care organizations, Ken Cess rejoined the company as a Regional Director of Operations in 2011 to oversee operations at Windsor’s 7 Northern California facilities. By implementing myriad strategies and developing innovative employee and consumer programs, Mr. Cess has successfully increased quality care and improved efficiencies for many multi-site facility operations. Mr. Cess attended the University of Illinois and has held positions as chairman and president for various CAHF chapters including GR Reimbursement Subcomittee as well as a member of the Board of Directors for the Central Coast Commission of Senior Citizens.

With an extensive background in California’s skilled nursing care community, Ms. Robinson is a leader in administrative procedures and quality care. As a licensed administrator and a regional director of operations, Ms. Robinson oversees operations at Windsor’s ten Central California facilities. In this capacity, she expands business relationships within the medical community, contributes to Windsor’s regulatory implementation, and provides guidance to administrators and the regional team.
David Farrell David Farrell
Regional Director of Operations, Northern California
In addition to his role in Operations, Mr. Farrell serves as Director of Organizational Development and steers Windsor through the Centers for Medicare Services’ nursing home five-star rating system. His strong background in organizational culture and process enhancement has contributed significantly to Windsor’s quality improvements. Mr. Farrell, who holds a Master’s degree in social work, with a concentration in gerontology and administration from Boston College, is a licensed Nursing Home Administrator. Recognized as a leader in nursing rehabilitation, Windsor’s skilled nursing and rehabilitation centers partner with physical, occupational and speech therapists, dedicated to helping residents achieve their highest level of independence. Collaborating with physicians and utilizing the latest equipment and techniques within a supportive atmosphere, each Windsor facility implements comprehensive, multi-tiered and customized nursing rehabilitation programs tailored to each resident’s specific needs and goals.

Windsor’s ultimate goal is to safely return residents to their homes, or other appropriate living arrangements, as soon as possible. Nursing Home Rating System

The Centers for Medicare and Medicaid Services (CMS) designed a comparison system to help prospective patients, families and caregivers select the right long-term care facility. This is known as the Five Star Quality Rating System.

It’s good news that today’s skilled nursing and rehabilitation centers differ greatly from the conventional nursing homes of the past. At Windsor, you will find residents who require long-term care, but you are equally likely to meet residents who stay briefly, depending on the nature of their illness, injury or rehabilitation needs.

There are many important resources available for prospective residents or families who are seeking care for themselves or a loved one, and CMS data may be useful. However, no rating system can address all of the important considerations that go into making a decision about which skilled nursing facility is best for you or your loved one. You are encouraged to take all available resources into account as you consider your options. Just as it is impossible to buy a car without kicking the tires, it is impossible to make the right healthcare choice without personally visiting Windsor. We encourage you to tour our facility and meet our staff and residents before making a decision.

A La Carte Dining at Windsor Elmhaven Care Center

Imagine starting the day with your favorite omelet, muffins, coffee and juice. It may sound like a visit to your neighborhood coffee shop, but it’s just another Monday morning for Windsor Elmhaven Care Center’s residents.

Windsor’s “Morning Choice Program” offers residents made-to-order specialty omelets cooked table-side with their choice of ingredients. Dietary Supervisor and Chef, Doug Hankins, created the program to endow residents with choice over their dining experience, something old-fashioned skilled nursing centers rarely provide. Having a meal made to order is a luxury in this setting, and residents have enthusiastically welcomed this personal touch.

Given its popular response, Hankins expanded the program with a Wednesday Pasta Party! Residents choose their sauce, meat or vegetables, which are prepared right before their eyes. The Pasta Party has become such a lunchtime favorite that there is more to come. A dinner program is in the works, offering Windsor’s residents more choices of freshly made-to-order meals.

Certified Nursing Assistants – The Heart of Person-Centered Care

The following article was written by David Farrell, MSW, Windsor’s Regional Director of Operations.

Oakland’s Skilled Nursing Facilities (Nursing Homes) continue to play an important role in our local healthcare continuum. In fact, those over the age of 70 have a good chance of experiencing a nursing home firsthand. But…their stay is likely to be a short one. The rehabilitation and nursing care gets most back on their feet and into the community within 30 days. Oftentimes, the difference between a dreadful stay in a nursing home and a wonderful stay depends on an employee you might not immediately think of – the certified nursing assistant (C.N.A.).

Next time you visit a nursing facility, pay attention to the C.N.A.s. These unique caregivers deliver the majority of hands-on care in skilled nursing facilities. Most are masters at their skilled trade. Compassionate and strong, the very best deliver “person-centered care.” They tune in to the unique daily pleasures of the elders they serve. And as a result, they just don’t give care…they give life.

At Windsor of Oakland, we practice consistent assignment of our C.N.A.s. That means that each one of our elders is primarily served by 3 C.N.A.s five days a week, 24 hours per day. These three C.N.A.s are consistently relieved on their days off by 3 other C.N.A.s. This staffing model allows these six C.N.A.s to really get to know the individual patients. This staffing model helps to facilitate compassionate, individualized care.

Loved ones and family members of elders in skilled nursing facilities can also help to promote person-centered care. Bring in photographs and educate the C.N.A.s about their patient’s life before entering the nursing home. Taking the time to tell them about your loved ones life will provide the C.N.A.s with a deeper perspective leading to more respectful care. Also, the C.N.A.s really appreciate when you notice and compliment them for their efforts. It means a lot coming from all of you. And, by noticing and complimenting the C.N.A.s, you will help to keep the morale up.

Community Relations – The Ridge Shows How Much They Care.

Congratulations to Windsor The Ridge Rehabilitation Center in Salinas who was recently honored at CAHF’s Legislative Conference with a Showcase Award.  Showcase Awards were created by CAHF in 2006 by the Public Advocacy Committee in order to recognize facilities, chapters and regions that create positive awareness of long-term care.  Under the Community Events category of the Showcase Award, The Ridge won for their Toys for Tots program benefiting the children of Monterey County.  In this Windsor Best Practice, we highlight the 2011 Windsor Administrator of the YearRachyl Bruton, Activity Director Pam Hernandez and their team for showing how much you care beyond the walls of the Windsor The Ridge.

Showcase Awards nominations are judged by the Public Advocacy Committee on the criteria of creativity of the event, ease of replication for other providers and how the event generated positive attention for long-term care.  Windsor The Ridge stood out in front of the other nominees because of the volume of toys they donated and the fun they had in the process.  And last December, they made a huge positive impact for hundreds of under privileged kids in Monterey County during the holiday season.

Windsor The Ridge developed a simple concept to collect toys.  They started fundraising early in the year and they coordinated a free, outdoor concert and attendees who brought a toy got a ticket for a free food item.  The Ridge had a live Mexican band that donated their time and played for 2 hours. Their residents greeted the public as they entered the concert area. Volunteers covered the concession stands. Everyone ate, danced and had a great time.

Activity Director Pam Hernandez said, “The Activities Department could not have put this great event together without the help of all the volunteers… staff, their children, the residents and residents’ families. Their loving hands and generous hearts made this event successful.”

Every month, the Activity Department held luncheon fundraisers.  In October, they sold a Mexican plate dish.  In November, it was an Italian plate dish.  And their generous, caring staff and the residents’ families continued to bring in toys.  By December, they were able to raise more than $2,500 and purchased 375 toys for the kids.  They loaded up the toys and a resident in Rachyl’s red pick-up truck and their delivery was publicized on the local KSBW-TV news broadcast.

When your organization gets involved in the community-at-large, it enhances the quality of life of the people you serve, the staff who serve them and your reputation.  When you reinforce your organization’s commitment of care to the broader community, you become the provider of choice.  Caring produces results!

Continuum of Care Event

Four Windsor skilled nursing centers were proud co-sponsors of a recent Continuum of Care dinner held for 30 Salinas area cardiologists, internists and general practitioners. Held at Monterey’s Intercontinental Hotel, guests heard keynote speaker Manny Villafana, Chairman and CEO of Kips Bay Medical, trace the transition from the initial heart valve to valves used today. Mr. Villafana also discussed his latest contribution, the eSVS Mesh system for CABG patients.

Windsor is a key player in the Continuum of Care program, partnering with Philips Respironics, Family in Home Caregiving, Alliance Home Health and Care Medical to provide quality and excellence in patient healthcare.

250,000 Americans annually receive a life saving medical device developed by Villafana. An entrepreneur, having first founded a company that revolutionized the pacemaker industry by introducing a long life lithium iodine pacemaker, Villafana subsequently founded St. Jude Medical, Inc., where he led their development of a novel Nitinol heart valve that dominates the mechanical valve replacement market; an exciting development that reduces repeat surgeries.

Five Windsor Facilities Recognized by AHCA/NCAL National Quality Award Program

Windsor is proud to announce that five of its Southern California facilities were awarded the Bronze Quality Awards from AHCA/NCAL based on the program submissions earlier this year.

The AHCA/NCAL National Quality Award Program provides a pathway for providers of long term care services to journey towards performance excellence. The program is based on the core values and criteria of the Baldrige Performance Excellence Program

The five facilities who earned the Bronze Quality Award are:

  • Windsor Palms of Artesia
  • Windsor Gardens of Fullerton
  • Windsor Gardens of Long Beach
  • Windsor Gardens of Los Angeles
  • Windsor Gardens of San Diego

The Quality Award program has three progressive step levels. Applications are judged by trained Examiners who provide feedback on opportunities for improvement to support continuous learning. Facilities must achieve an award at each level to progress to the next level.

Bronze – Commitment to Quality applicants begin their quality journey by developing an organizational profile including vision and mission statements, an awareness of their environment and customers’ expectations, and a demonstration of their ability to improve a process.

“Last year we had only one recipient in our Region and two for all of Windsor who received this award,” says Kevin Bowman, Windsor’s Southern California Regional Director of Operations.  ”This year, almost half of our Region is being recognized. It is a great testament to our local Administrators and their teams who show their commitment to quality processes and outcomes.”

I am writing this unsolicited letter to you in recognition of the exemplary work done by Mr. Ross Maultasch and his staff at the Windsor Garden Care Center of Fullerton.

My mother, Virginia Bonsangue, 85, has been a patient there since early December following abdominal surgery and other related issues. I met Ross when she had a concern regarding one of the night staff members during her first week. Ross took the concern seriously addressed it in a professional and timely manner.

Our experience at each level, from admissions to RN/CFN to PT/OT, has proven to be positive and effective. I am especially appreciative of the patience the medical staff have shown towards Virginia, who is a large person and not easy to handle.

As one who manages more than 80 part-time faculty members and teaching associates, I understand the commitment and time it must take to run a good Care Center. My family and I are deeply appreciative of Ross Maultasch’s effective leadership and genuine commitment, as well as that of the staff at WGCCF, to providing quality patient care.

My name is Hannah Switzer. I recently spent approximately two weeks in your facility for physical therapy. I have worked in the medical profession all my life. Every facility such as yours were so repulsive I would not send anyone there. I had to see a dear lady in one of these places and it smelled so strong of urine it made me nauseated.

When I was in Windsor Gardens [of the Valley] I had a very pleasant surprise.

My compliments go to Rose Banda (day shift) and Maria (night shift). They were patient with me and the same can be said with all the staff and physical therapy staff.

There is one “personality” who deserves a special compliment.  This “individual” is called “Mr. Windsor.” He always seemed to know when I needed special attention.

One more thing – I love your patios, especially the one with the dove fountain.

If I ever need residential care, I will call Windsor Gardens. Thank you and your staff for a very pleasant stay and a change of feelings towards facilities like yours.

Windsor Announces Launch of Electronic Health Care Records Program

Windsor Healthcare recently announced the launch of an electronic health records (EHR) program in eight of their California facilities, as part of the Medicare and Medicaid EHR Incentive Program. The EHR Incentive Program provides incentive payments to eligible professionals, eligible hospitals and critical access hospitals (CAHs) as they adopt, implement, upgrade or demonstrate meaningful use of certified EHR technology.

Leaders in eight Windsor facilities spent the month of March training their licensed nurses, certified nurse assistants and department managers on the use of the system to go live on 4/1/12. The eight locations – Concord, Hayward Gardens, Fremont, Valley, Artesia, Terrace, Fullerton, and Salinas - implemented the usage of electronic health records in an effort to decrease errors and provide better care.

“We hope that the new program will help us remain in compliance with the state and federal regulations,” says Marissa Kirby, Administrator at Windsor Gardens Healthcare Center of the Valley, “It will also help us reduce medical errors, decrease duplicative documentation, and streamline care for our patients and residents.”

Windsor Vallejo Recognized by ACHCA as a Long Term Care Leader

Windsor Vallejo Nursing & Rehabilitation Center was recently recognized by the American College of Health Care Administrators (ACHCA) as a top performing Skilled Nursing Facility. For 2012, CMS’s MDS quality indicator and three years of suvey data were the basis on which Windsor Vallejo was identified as a leader in the long term care industry.

Specifically, ACHCA recognized Windsor Vallejo for excelling in the following four areas:

  1. Health Survey
  2. Fire (Life Safety) Survey
  3. Complaint Surveys
  4. Improvement in the last two quarters of the reported CMS MDS Quality Measures.

By continually addressing the complete needs of each resident, the Windsor approach changes the entire nature of the caring environment.
At Windsor Redding Care Center, we’re proud of our innovative approach to sensitive and effective care. In fact, our sister Windsor facilities have hosted professionals from USC and UCLA who wished to see Windsor’s pathbreaking protocols in action so they could teach others how to apply them elsewhere.
Discovery Program – In-house, forty hours per week, we conduct sensory and tactile stimulation for low-functioning and dementia residents.
Intervention – Mind, Body, and Soul – A behavioral program designed for aggressive intervention before – and in addition to – psychotropics. This individual and group therapy is designed for the geriatric and our younger population.
In conjunction, we also offer free family support groups and care-giver support groups led by a psychologist and our own MSW.
Hydration Program – We regularly have a C.N.A. whose sole purpose is to provide water to the residents and to chart it.
Transparency – Family Night – We make sure that, on a regular basis, families can speak with the administrator and learn about everything happening here.

Subacute Care Providers

Subacute care bridges the gap by joining the sophisticated technology of a hospital with the efficiency of a skilled nursing facility. In our separate dedicated subacute units, Windsor’s specially trained interdisciplinary professionals lead the way in subacute care. We treat medically fragile patients with advanced technologies, sophisticated therapies and equipment, as well as compassionate patient-centered care. We provide services such as ventilator therapy, inhalation therapy, tracheotomy care, intravenous therapy, tube feeding, and complex wound care. Our mission is to arrest the subacute patient’s disease process, stabilize them, so they may return safely to their lives as soon as possible. Severe injury or illness often affects the whole person - body, mind and spirit. In caring surroundings where residents and families experience the comforts of home, Windsor’s nurses, therapists, dietitians and case managers also offer social and spiritual services as well as enrichment activities. Windsor currently offers three Subacute Care Centers in California:
